Your First Visit

Welcome, on behalf of the whole Coho Dental team! For your convenience, our new patient forms are available for you to print and fill out in the comfort of your own home. If you would prefer to complete your forms in the waiting room, please plan to arrive 15 minutes early.

If you have dental insurance, please bring your insurance card and a valid form of I.D.

Payment Options

Our office accepts a variety of payment methods to keep our care accessible and affordable for our patients. We accept most major debit and credit cards and offer a 5% discount to patients paying with cash and seniors. Patients can also use their FSAs, HSAs, or CareCredit® payment plans to cover the cost of their care. Coho Dental is an insurance-friendly dental practice, and we are happy to help our patients file, track, and update their insurance claims to help them maximize their benefits to reduce out-of-pocket expenses.

CareCredit^® Financing

Uninsured or underinsured? CareCredit offers a convenient way for patients to finance the cost of their dental care. CareCredit provides no or low-interest payment plans that can be designed around each patient’s monthly budget.
